Our web host also processes standard technical information that every website receives, such as your IP address, browser type and the pages you requested. This is used for security and to keep the site running.

What this website does not collect

The forms on this site deliberately do not ask for diagnoses, medications, medical history, symptom details, laboratory results, Social Security numbers or insurance information.
